- Enrollment number and program name
- Category (SC/ST/OBC/PwD/others)
- Annual family income
- Residential and contact details
- Attach clear photocopies of the following documents:
- Date of birth proof (e.g., birth certificate or 10th mark sheet)
- Aadhaar card and address proof
- Educational certificates (10th, 12th, graduation if applicable)
